How to get and use Sacrificial Heart in Path of Exile 2

In Act 3 of Path of Exile 2 there is a side quest you can do involving a Sacrificial Heart that will earn you two Weapon Set Passive Skill Points making it extremely worthwhile to do. Here is how to get and use the Sacrificial Heart in Path of Exile 2 and get the extra two passive skill points.

Where to find the Sacrificial Heart

The entirety of this quest takes place in Act 3 on both Normal and Cruel difficulties in PoE2, specifically in the drowned city called Aggorat. Once there, start killing any enemies you come across, as each one on this map specifically has a chance of dropping the Sacrificial Heart. When dropped, the item will show up with bright green text, so it should be easy to spot and pick up. There’s not a specific enemy that drops it, however, you are guaranteed to get the Sacrificial Heart as long as you kill every enemy on the Aggorat map.

After picking up the Sacrificial Heart, it will stay in your inventory until you complete the quest taking up a 2×2 grid. As such, you should try to make an effort to complete this quest while in Aggorat both for the reward but also to free up the valuable inventory space.

How to use the Sacrificial Heart

With the Sacrificial Heart in hand, it’s time to perform the ritual. The ritual area can be found near the end of the Aggorate zone, usually on the northeast side of the map somewhat close to the Black Chambers exit of Aggorat. If you look on your minimap as you explore, the area to complete this quest and use the Sacrificial Heart will be labeled ‘Blood Sacrifice’.

At the actual Blood Sacrifice area, you will see a platform with two key pieces of text: a green-text Sacrificial Dagger item, and a white-text Place Sacrificial Heart action. You first need to click the white text, which will place the Sacrificial Heart on the altar. Then, pick up the Sacrificial Dagger, and click on the newly appeared white-text labeled Stab Sacrificial Heart. Your character will perform the quick stabbing ritual, and this quest will be completed.

Sacrificial Heart quest rewards

After stabbing the Sacrificial Heart at the Blood Sacrifice Area, you will immediately receive two Weapon Set Passive Skill Points, and two Passive Skill Points, meaning if you ever change weapons you will receive those two points. Passive Skill Points are extremely valuable and are typically obtained via leveling up but can also be acquired by completing specific quests in each of the acts.

Interestingly enough, this quest will appear on both Normal and Cruel difficulties. The quest works the same way with you having to find and collect the Sacrificial Heart, and then stab it at the Blood Sacrifice area. However, you will receive the two Weapon Set Passive Skill Points and two Passive Skill Points on both difficulties meaning you can get four of each point in total by completing this quest.

Furthermore, this is the only quest in Act 3 Normal and Act 3 Cruel in which you can get the two points. The other two acts on both difficulty have their own single quests which can be completed for them as well, meaning in total you can receive 12 Passive Skill Points and 12 Weapon Set Passive Skill Points for completing specific side quests across all three Acts and both difficulties.
