Baldur’s Gate 3 is full of main quests and side quests, but one of the central points of the story is finding out how to remove the Parasite. The cRPG spans over 100 hours, so it can be a bit overwhelming trying to manage all your quests. There are also often many ways to complete each quest, including your quest to Remove the Parasite. Here is how to remove the Parasite in Baldur’s Gate 3.

Obviously, there are some spoilers here as removing the parasite is part of the main storyline. If you want to know whether or not you can actually remove the Parasite as in the Illithid Tadpole Parasites in your brain, this is the information you were looking for.

Unfortunately, you cannot actually remove the Parasite in BG3. Depending on the choices you make a long the way, there are different outcomes in regard to the Illithid Tadpole infestation in your brain. However, none of these outcomes result in you actually removing the Parasite.

In fact, via one outcome you can destroy the source of power for the Illithid Tadpoles, but even then you do not remove them. Instead, the Parasite simply dies while still in your brain, and you no longer get their powers.

How to complete the Remove the Parasite quest in BG3

Over the course of BG3 you will have a quest called Remove the Parasite that will update with various methods you can try to complete it. Here are all the different ways you can attempt to remove the Parasite across Baldur’s Gate 3.

First, you can visit the Druid Healer named Nettie in the Inner Sanctum. Unfortunately, her cure is actually killing you with poison, but you can use Perception and Persuasion to get out of the situation and get a new lead which is to track down the druid Halsin.

You’ll find Halsin in the Goblin Camp, and be tasked with freeing him. He’s in bear form, so don’t attack the bear when you encounter it, but instead help it escape. Halsin won’t be able to remove the Parasite either, but he will join your party.

If you explore the Riverside Teahouse in the Sunset Wetlands in Act 1 you will encounter Auntie Ethel. If you tell her about the Parasite, she will offer to remove it but requires your eye in exchange for doing so. Even if this sounds like a worthy trade to you, I wouldn’t do it. In the end, Ethel will fail to remove the Parasite, and you will still lose your eye which grants you a debuff that lowers Perception checks against Hags.

Also at the Goblin Camp you will encounter Sazza, the Goblin Priestess. If you ask her to remove the Parasite, she will imprison you in the Defiled Temple, which you will then have to fight your way out of.

Finally, you can rescue Volo from the Goblin Camp who will then join your camp. He offers to remove the parasite using a very primitive medical procedure where he attempts to jab a pick through your eye and into your brain. You’ll also lose your eye in this situation, and, which should be no surprise, will not remove the Parasite. However, you will gain a buff that grants you See Invisibility, allowing you to see invisible enemies at all times.
